Search Request Form You may print this form to request an Indiana and Ohio Directories Search. The fee for one search is $5.00. Two or more searches requested at the same time are $3.00 each. Each surname and directory request counts as one search. You may list alternate spellings for the surname and they will be searched also and count as one search. Please list the surname, state, directory code and search fee for each search requested. The directory codes are the eight or nine character codes in brackets listed on the IN Directories and OH Directories pages. Please allow two to four weeks processing time.
